Registration Requirements & Instructions
Please complete and mail or fax to NPFOCC office prior to the start of each class session! Registration and payments will not be accepted by the instructor or facility. The donation cost for this session is $5.00 per class. Participants are required to pay the fee in full, payable in advance, as payments per class will not be accepted, regardless of attendance or when started or stopped. This fee is for the Parkinson patient only. There is no fee for the caregiver if one is needed to assist with the patient.
